BEDFORDSHIRE & LUTON REFERRAL FORMS
CHUMS have been working with our partners ELFT CAMHS to support referrals into our Emotional Wellbeing Service.
To ensure your referral is safely managed and allocated into the most appropriate care pathway for the presenting needs, all referrals are redirected into a Single Point of Entry (SPOE) for triage and multi-agency decision making. This may mean some cases may be allocated back in to CHUMS for a piece of work or signposted to other support routes.
To Make A Referral: Click on the appropriate link below for your area. This link will redirect you to your local CAMHS website where you can find the relevant referral form (Professional or Self Referral) under “Information Leaflets” on the right-hand side of the page.
PLEASE NOTE: Parent/Carer referrals can be made using the self-referral form.

What happens to your personal data?
The information that you share with us stays within a dedicated computer system and is stored on a specialist client database called PCMIS. This information does not stay within our website and submitted referrals are not stored within the website environment or with any other third party. Referrals are transmitted directly into our database. This ensures that personal data is stored safely and securely in accordance with the Data Protection Act 2018. This is only accessed and used to plan and monitor CHUMS support.
We regularly share information with other professionals including CAMHS and Early Help. This is to ensure that we are working together to provide every young person with the right support at the right time. Information is shared in accordance with guidance from GDPR 2018 (Article 6) under the criteria of ‘vital interest’ and ‘legitimate interest’. By completing the referral form you are agreeing to the above.
